This pilot project will run to the end of 2025 to assess the interest and feasibility of providing a shortened version of the uSPEQ Consumer Experience Survey, and determine:

  • Whether the shortened version captures key information to be useful for program planning.
  • Whether organizations find the survey results useful for their quality improvement.

The uSPEQ Mini survey is an abridged version of the uSPEQ Consumer Experience Survey with demonstrated validity and reliability, used by hundreds of service providers for quality improvement. The survey:

  • Contains eight questions across four domains, including service access, effectiveness, outcomes, and service experience/satisfaction of persons served.
  • Responses are confidential and anonymous.
  • Is available in English and Spanish.

Participating organizations may begin collecting data immediately, real-time results available through the end of 2025. This project will include a limited number of free surveys available on a first-come, first-serve basis; secure your spot by signing up early!

View Pilot Project orientation video

  • To familiarize you with the uSPEQ Mini Survey pilot project, you will be sent a link to an orientation video, along with the information you will need to start your data collection.

Distribute questionaires

  • This is an online survey only. uSPEQ will provide you with a URL link and a QR code to the survey to distribute to consumers. Access is available by any device connected to Wi-Fi.
  • Responses are submitted to the uSPEQ data collection system automatically.

View survey results

  • uSPEQ will provide a link to a dashboard where response results may be seen in real time.

Provide feedback

  • At the conclusion of the pilot project in December 2025, you will be asked to complete a brief questionnaire regarding your experience with the uSPEQ Mini survey.

Organizations are not required to use uSPEQ services to receive accreditation, and utilizing uSPEQ does not guarantee accreditation.

  • uSPEQ is a department within CARF International that offers services to assist organizations in collecting feedback from their stakeholders.
  • Although uSPEQ is part of CARF, the department provides services separate from accreditation. It is up to the organization to decide whether it wants to share the uSPEQ survey results with CARF surveyors during their accreditation visit.
