Other Stakeholder Surveys

In addition to survey tools for persons served and employees, uSPEQ provides surveys for many other types of individuals or groups who may be directly involved with your organization’s services.

  • Allows family members to share their perceptions of their relative’s care
  • May be used in a variety of settings, including residential services, aging services, medical rehabilitation, or programs for behavioral health, mental health and substance use disorders
  • Allows persons with intellectual or developmental disabilities to share their experience of the care they receive
  • Survey items are written in simple language and responses may be provided on a pictorial rating scale (sad face/happy face)
  • May be filled out independently or administered via individual interview as respondents’ capabilities allows
  • Self-assessment for board members to assess the board’s effectiveness and efficiency, relationship with organization management, and provide feedback in areas such as board structure, meetings, and organizational directions
  • May also be used to determine areas of expertise needed for future board members
  • Gathers feedback from community groups that work with your organization to provide referrals, resources, or additional support to persons served
  • Respondents are able to provide feedback on their working relationship with your organization (trust, communication, etc.), perception of effectiveness (“this organization meets the needs of the persons with disabilities we refer”), and make suggestions for future partnerships
  • Rather than conducting a longer, more comprehensive annual employee feedback survey, organizations may choose to offer shorter “check in” surveys on a quarterly basis
  • These surveys generally have between three and five questions as well as one or more comment boxes
  • Employees may be more willing to provide honest feedback on a two-minute survey than a more detailed feedback form
  • Allows your organization to determine employees’ current concerns, take action, then assess satisfaction soon thereafter
  • uSPEQ staff are able to conduct completely custom surveys to meet your organization’s needs, developing methodologically sound questionnaires to assess the concerns of any population involved with your organization
